GLP-1 Muscle Preservation Quick Start Guide | Ernest Health
Ernest Health GLP-1 Quick Start Guide
This guide is general education, not individual medical advice. Any change to your GLP 1 dose, calorie target, or training load should be reviewed with your prescribing clinician.

GLP 1 medications drop appetite fast. Without a plan, you lose fat and muscle together, which slows your metabolism and works against you long term. This guide covers the essentials. It is not the full plan, but it will get you moving in the right direction today.

The Evidence: Medication Plus Training

Research on GLP 1 therapy increasingly supports pairing medication with structured training rather than relying on medication alone.

  • A recent network meta-analysis in Obesity Reviews found that combining GLP 1 therapy with structured exercise produced the largest improvements in body weight, fat mass, and waist to hip ratio compared with placebo.
  • A systematic review in Annals of Internal Medicine, covering 38 randomized controlled trials and 25,816 adults without diabetes, found placebo adjusted weight loss reaching about 19 percent with tirzepatide, 14.8 percent with injectable semaglutide, and 12.4 percent with orforglipron.

GLP 1 therapy creates the opportunity to improve body composition. It does not replace resistance training, adequate protein, and preserved physical capacity, those are what make the results last.

1. Protein Comes First

The target: Aim for roughly 0.9 to 1.1 grams of protein per pound of your target body weight each day. On a GLP 1, that number matters more than it ever has, because appetite is working against you.

The habit that matters most: Spread protein across 4 feedings a day instead of 1 or 2 large meals. A body under 40 grams of protein does not trigger the same muscle building signal as one that clears it.

  • Start the day with a liquid protein shake. It goes down easy even with low appetite.
  • Keep a protein source within reach for the afternoon slump, when most people skip meals entirely.
  • If you can only manage two meals some days, make both of them count: 40 grams or more each.

2. Lift, Do Not Just Walk

Cardio alone will not protect your muscle on a GLP 1. Resistance training is what tells your body to keep the muscle you have while the fat comes off.

  • Train full body, 3 days a week, with at least one rest day between sessions.
  • Focus on the big movements: a squat or leg press pattern, a hinge like a deadlift or hip thrust, a push, and a pull, every session.
  • Leave 1 to 2 reps in the tank on every set. This is not the time to train to failure while eating less.

3. The One Warning Sign To Watch

If your strength in the gym drops noticeably over 2 weeks while the scale keeps moving, that is usually a sign you are losing muscle, not just fat. This is common on GLP 1s and it is fixable fast if you catch it early. Most people do not catch it until months later, when a scan finally shows the damage.
